Can your spouse fill a harem with all the different kinds of lovers they are using in place of a proper marriage?  Unfortunately, when a spouse has unmet needs, they will look elsewhere rather than communicate with you openly and honestly.  You may even find that they will tell you about one issue, when there are many more lurking just beneath the surface.
